Michael M. Gutierrez, M.D.
Dr. Gutierrez came to Orlando in 1987 where he has been in private practice for nearly 20 years. He was Chief of Dermatology at Florida Hospital from 2000 to 2003 and is currently on staff at the Orlando Regional Medical Center and the University of Central Florida's Medical Center. His professional memberships include the American Academy of Dermatology, the American Medical Association, the Central Florida Society of Dermatology and the Florida Society of Dermatologic Surgeons. He was a board member of the Pan American Medical Association and a counselor for the Southern Medical Association.
Dr. Michael Gutierrez was born in Cuba in 1957. He immigrated to the Unites States in 1960 where he grew up in New Orleans, Louisiana and Mount Claire, New Jersey. He is married to Annalisa his wife of 25 years and has three daughters.
Dr. Gutierrez did his undergraduate education at the University of New Orleans where he received a Bachelor of Science in Biology. He did his medical training at Louisiana State University Medical School and completed his degree in 1983. He did an internship at Charity Hospital in New Orleans LSU division and his residency at LSU Charity Hospital New Orleans. He was Board Certified in Dermatology in 1987. During his residency at LSU, Dr. Gutierrez was recipient of the Peterkin Award for the Outsanding Resident Research Projects for 1987 and also received a Continuing Medical Education Award from the American Academy of Dermatology during that time.
Dr. Michael. Gutierrez is Board Certified in Dermatology and Dermatological Surgery.

Oscar Saffold, M.D.
Dr. Oscar Saffold joins us after practicing Dermatology and Dermatological Surgery for over 30 years in Cleveland, OH. He is a former Assistant Clinical Professor of Dermatology at Case Western Reserve University School of Medicine.
Dr. Saffold received his Dermatology training at Tufts University in Boston, MA and is Board Certified in Dermatology.
His professional experience has been distinguished not only in his medical profession, but in philanthropy and numerous leadership roles in health maintenance.
Dr. Saffold sees patients at Ashley Park in Metro West and has been with our practice since October 2006.

James L. Baker, M.D.
Dr. Baker is a Board Certified Plastic Surgeon who specializes in Cosmetic Surgery. He is a Traveling Professor for and a Past President of the American Society for Aesthetic Plastic Surgery as well as a Past President of The Florida Society of Plastic Surgeons. He is the Immediate Past Chairman of the Plastic Surgery Department for the Florida Hospital System and is also a Clinical Professor of Plastic Surgery at the University of South Florida, Tampa.
Dr. Baker has been in private practice since 1971, has appeared on The Phil Donahue Show, The Oprah Winfrey Show, “20/20” and has been named one of the best in his field in such magazines as Town and Country, Harper’s Bazaar, Mirabella and Self. He has been named “Best Plastic Surgeon” by the readers of Orlando Magazine and has also been listed in “The Best Doctor’s in America” numerous times. Dale G. Salatich, M.D.
Dale G. Salatich, MD is board certified in Otolaryngology and Head and Neck Surgery. Dr. Salatich earned his undergraduate degree at the Louisiana State University in 1979. He then continued on at Louisiana State University earning his medical degree in 1983. He spent his Internship year at Earl K Long Memorial Hospital in Baton Rouge, Louisiana. Dr. Salatich then returned to New Orleans and completed a four-year residency in Otolaryngology at Charity Hospital. Dr. Salatich moved to Orlando in 1989 where he worked in otolaryngology private practice until 2008. He is currently an assistant Professor at the University of Florida School of Medicine where he has instructed ENT residents for the past five years. Dr. Salatich specializes in excision and reconstruction of head and neck skin cancer. He joined Mid Florida Dermatology Associates in September of 2008 and limits his practice to excisions of head and neck tumors.

Fitzgeraldo A. Sánchez, M.D.
Dr. Sánchez completed his dermatopathology training at the Ackerman Academy of Dermatopathology in New York. City. Prior to this he was the Smith Gibson research fellow at the Mayo Clinic in Rochester, Minnesota. During his time at the Mayo Clinic he focused his research on B-Raf mutations in advanced level melanoma. His dermatology training was performed at the Skin and Cancer Unit of the New York University School of Medicine in New York City where he also completed his internal medicine internship. His undergraduate studies were performed at Boston University and at Goshen College.
The medical education of Dr. Sánchez was completed at the Ponce School of Medicine in Puerto Rico, and during that time he completed research projects in Hansen’s disease at the University of Puerto Rico and in confocal microscopy at the Wellman Laboratories of Photo-Medicine at the Massachusetts General Hospital. Dr. Sánchez has published many articles and book chapters on microscopic techniques and fillers. He is on the editorial board of the Journal of Drugs in Dermatology, reviews articles for the American Journal of Dermatopathology and is an active member of the American Academy of Dermatology’s Health Care Finance Committee, the Puerto Rico Dermatological Society and the Paul O’Leary Society.
Dr. Sánchez’s expertise in dermatopathology will permit Mid-Florida Dermatology to expand its services to our patient population. He is well versed in English and Spanish and is very enthusiastic about serving all our communities. Our goals are to offer the best patient care in all areas of dermatology, including dermatopathology, and with his training he will add an excellent foundation that we will translate into better patient care. It is our mission together with Dr. Sánchez’s to provide an environment where the patient care is always first.

Ayyaz M. Shah D.O.
Dr. Shah joined Mid Florida Dermatology Associates in October 2007. He has been practicing dermatology for nearly 10 years. He has practiced dermatology in the Mid West and was on staff at five hospitals in Iowa which include Covenant Medical Center and Allen Hospital. Dr. Shah was a member of the Executive Committee at Allen Hospital. His professional memberships include the American Osteopathic Association , American Association of Physician Specialties and The American Academy of Cosmetic Surgery.
Dr. Ayyaz Shah was born in Pakistan in 1966. He immigrated to the United States in 1977 where he grew up in upper New York and New York City. He is married to Humaira, his wife of 18 years and has three sons and four daughters.
Dr. Shah did his undergraduate education at City University of New York-Lehman College, where he received a Bachelor of Science degree in Chemistry. He did his medical training at New York College of Osteopathic Medicine and completed his degree in 1994. He completed an internship at St. Barnasar Hospital? in Bronx, New York. He initially completed a family practice residency in 1997 at Wyckoff Heights Medical Center in Brooklyn, New York and then completed his Dermatology training in the Mid West at Advanced Dermatology and Laser Center/Universal Medical Center in 2000. He is dual Board Certified in Family Practice and Dermatology.

Laura L. Mays, PA-C
Laura L. Mays is a nationally certified physician assistant licensed by the Florida Board of Medicine. Laura has over 12 years of healthcare experience with 8 years specializing in dermatology.. She specializes in moles/melanoma and skin cancer screenings, but treats patients with eczema, psoriasis, warts, acne, and rashes. Laura also provides Botox services to her patients who are wanting to turn back the hands of time.
In 1997, during her sister’s battle with melanoma, she created the Cynthia M. Disman Melanoma Initiative (a fund of MD Anderson Cancer Center Orlando), a non-profit organization that focuses on melanoma awareness and fund raising. Ms. Mays received national recognition for her work in melanoma from the American Academy of Dermatology (AAD) in July 2004 with a Gold Triangle Award.
Currently, she is working with lobbyists to pass “Cindy’s Safe Sun Bill.” She wrote the bill to ensure that all Florida school children be educated on moles, melanoma, skin cancer, and sun protection every year. Because of her efforts in the Florida Senate SB 772 (Effective July 1, 2006) states that every child in Florida is entitled to use sun protective items (sun block, hats, etc) when outdoors on campus. Previously children were not allowed to wear these items while outdoors on campus.
While writing dermatology articles in local magazines, Ms. Mays enjoys conducting melanoma lectures to corporations, schools, universities, and community groups. In July 2006, Laura Mays was featured in a Channel 6 News report concerning the rising cases of skin cancer. Ms. Mays also formed the first online melanoma support group for central Florida to give those touched by the deadly disease an outlet for discussions and questions.
Ms. Mays benefits from participating as a preceptor to PA students nationwide and is a guest lecturer at the University of Central Florida’s Nurse Practitioners program. In addition, she is currently an adjunct professor and preceptor for the PA Program at Nova Southeastern University-Orlando.

Jason L. Welch, MSN, ARNP, NP-C
Jason L. Welch joined Mid-Florida Dermatology Associates with a very admirable background in medicine. With his diverse training as a combat medic for four years during Desert Storm and his five years working in various intensive care units and emergency rooms, Jason is highly quailified to provide the best care to our patients. He is a nationally certified by the American Academy of Nurse Practitioners and is licensed to practice through the Florida Board of Nursing with memberships in a variety medical associations. Jason specializes in general dermatology, dermatological surgery, and Botox enhancements.

Nitra H. Cole, MSN, ARNP, NP-C
Nitra H. Cole, MSN, ARNP, NP-C, is a nationally certified practitioner at Mid-Florida Dermatology specializing in general dermatology, dermatology surgery, Botox enhancements, and laser hair and vein removal.
Nitra received her Bachelor’s of Science in Nursing from Troy State University in 1998. Before working as a nurse practitioner, Nitra worked a s a critical care nurse at Florida Hospital where she also served in many leadership roles. Nitra graduated with honors from the University of Central Florida in 2006 with her Master of Science in Nursing and Family Nurse Practitioners Certificate. Nitra has worked closely with Dr. Michael Gutierrez and received extensive training in general dermatology, dermatological surgery.
Nitra is nationally certified by the American Academy of Nurse Practitioners and is licensed to practice through the Florida Board of Nursing. She is a member of the American Academy of Nurse Practitioners, the Central Florida Advanced Practice Nursing Counsel, the Nurse Practitioner Society of the Dermatology Nurses Association and the honor society Phi Kappa Phi.

Heather R. Brock, MSN, ARNP, NP-C
Heather R. Brock, MSN, ARNP, NP-C is a nationally certified family nurse practitioner and has worked in the healthcare field for the past 13 years. Heather is originally from Pensacola, Florida and received her Bachelor of Science degree in Nursing from the University of South Alabama in Mobile in 1996. After relocating to Orlando in 1997, Heather practiced critical care nursing in the emergency department and intensive care units as a staff nurse and eventually as a supervisor. In 2006, Heather graduated with honors from the University of Central Florida with a Master of Science Degree in nursing and is certified by the American Academy of Nurse Practitioners.
Heather joined Mid-Florida Dermatology in 2006 and trained closely with Dr. Michael Gutierrez. She specializes in general dermatology including the diagnosis and treatment of dermatologic conditions such as eczema, acne, psoriasis, warts, and skin cancers. Heather has also trained extensively in dermatological surgery.
Heather’s professional memberships include the American Academy of Nurse Practitioners and the Dermatology Nurses Association. -
Casey A. Nichols, MSN, ARNP-BC
Casey A. Nichols, MSN, ARNP-BC, is a nationally Board Certified Advanced Registered Nurse Practitioner. Casey has worked in health care since 1999.Casey joined Mid Florida Dermatology Associates in 2007 and has practiced closely with Dr. Michael Gutierrez and Dr. Oscar Saffold. She specializes in skin care, Botox enhancements, and performs dermatological surgeries. Casey shows genuine care and concern for her patients.Professional memberships include: Dermatology Nurses Association and Central Florida Advanced Practice Nursing Council. -
Brooke Gregory, MSN, ARNP, NP-C
Brooke A. Gregory, MSN, ARNP, NP-C is a nationally certified nurse practitioner with twelve years of health care experience. She is certified through the American Academy of Nurse Practitioners. Brooke joined Mid Florida Dermatology in 2008 and had been previously practicing as an ARNP in women’s health care in the Orlando Area for the past three and a half years.
Brooke received her ASRN degree in 1997 from the Florida Hospital College of Health Sciences and began working for Florida Hospital in 1997. Brooke received her Bachelor’s of Science in Nursing from the University of Central Florida in 1999. Before working as a nurse practitioner, Brooke worked as a critical care nurse at Florida Hospital where she also served in many leadership roles. She graduated with honors from the University of Central Florida in May of 2005 with her Master of Science in Nursing as a Family Nurse Practitioner. Brooke has worked closely with Dr. Michael Gutierrez and received extensive training in general dermatology and dermatologic surgery since joining Mid Florida Dermatology in 2008. -
Mickelle Riley, MSN, ARNP, NP-C
Mickelle Riley, orginally from Chicago, Illinois, graduated with Honors from the University of Central Florida with a Master of Science in Nursing in 2007. She is nationally certified by the American Academy of Nurse Practitioners as a Family Nurse Practitioner. Mickelle has been working in health care since 2003, with a background in emergency medicine and urgent care. She is a member of the Florida Nurse's Association and served as President for District VI of Florida Nurse's Association in 2008. Mickelle joined Mid Florida Dermatology Associates in 2008 and has trained and practiced closely with Dr. Michael Gutierrez receiving extensive training in general dermatology and dermatological surgery.

Abigail H. Plum, MSN, ARNP-BC
Abigail H. Plum, MSN, ARNP-BC, is a Registered Nurse Practitioner, nationally certified through the American Nurses Credentialing Center. Before joining Mid Florida Dermatology Associates, Abigail previously worked as a nurse practitioner specializing in endocrinology. Abigail also served as the nursing supervisor of a respite care center for Alzheimer’s patients. Abigail is a member of the American Nurses Association and Sigma Theta Tau International Honors Society of Nursing.
Abigail received her Bachelors of Nursing Science from University of Florida and continued her education at University of Florida to receive her Masters in Nursing Science in 2008. Since joining Mid Florida Dermatology Associates, Abigail has worked closely with Dr. Michael Gutierrez receiving extensive training in general dermatology and dermatologic surgery.

Evan P. Thady, MSN, ARNP-BC
Evan P. Thady, MSN, ARNP-BC, is a nationally certified nurse practitioner certified through the American Nurses Credentialing Center. Evan joined Mid Florida Dermatology Associates in 2009 and has been working closely with Dr. Michael Gutierrez receiving extensive training in general dermatology and dermatologic surgery. Before joining the practice, Evan worked in the field of women’s health as a research and clinical assistant and trainer for early breast cancer detection.
She worked directly with nursing faculty at the University of Florida training healthcare professionals skill-based breast examination techniques while assisting in medical research to improve the manual detection of breast cancer.
